Health Insurance: Town of Deerfield pays 65%, employee pays 35% of monthly premium
Dental Insurance: Employee pays 100% of monthly premium
Vision Insurance: Employee pays 100% of monthly premium
Basic $5000 Life Insurance plan through Boston Mutual: Town of Deerfield pays 65%, employee pays 35% of monthly premium

Employee's monthly cost x multiply by 12 (for the yearly cost) and divide by number of pay periods throughout the year.
Non-School Employees = 26 pays
School Employees = 22 pays (Deerfield does not take out deductions over the summer for school employees)
Example: HMO Single Plan Employee Cost = $352.45 / month
Non-School Employee = 352.45 x 12 = $4,229.40 / 26 pays = $162.68 / paycheck
School Employee = 352.45 x 12 = $4,229.40 / 22 pays = $192.26 / paycheck
